Health and Temperament Agreement
I agree and understand that in admitting my Paw to Max & Alayna’s Paws Play that my Paw is in good health, is current on all vaccinations and flea control and has not harmed or shown aggression or threatening behavior towards another Paw and/or human. I understand that in any cage-free Paw environment that there is an inherent risk of injury or illness from rough play and/or fights. Understanding this, I accept full responsibility and hold Max & Alayna’s Paws Play harmless for any pet injury, death or damage. I agree that I am solely responsible for any harm caused by my Paw while my Paw is in the care of Max & Alayna’s Paws Play. I agree not to hold Max & Alayna’s Paws Play and their associates liable for any injuries to my Paw while in the care of Max & Alayna’s Paws Play. I understand if my Paw shows repeated aggressive or menacing behavior that he/she will be moved to seclusion. If the behavior continues your Paw may be asked to leave Max & Alayna’s Paws Play. This is not a personal attack on the Paw, rather our attempt to ensure all of the Paws in our care are comfortable and kept safe. By signing this form, you acknowledge that you understand and accept the terms and conditions set forth by this agreement.
EMERGENCY MEDICAL TREATMENT AUTHORIZATION
Max & Alayna’s Paws Play will make every effort to contact you in any emergency situation with your Paw before we transfer to a licensed veterinarian. This authorization gives the associates of Max & Alayna’s Paws Play to act on my behalf in the event my Paw needs medical attention. I, the owner, authorize a licensed veterinarian and their assistants to administer treatment and perform procedures as are considered therapeutically and diagnostically necessary for the care of my Paw, including administration of anesthesia. In the event that emergency treatment is required, I authorize the veterinary staff and their assistants to perform medical and surgical treatments necessary to preserve the life of my Paw until I can be contacted for further approval. I accept full responsibility for any and all financial costs associated with the treatment that my Paw receives from the licensed veterinarian and their staff. I hereby release and hold harmless Max & Alayna’s Paws Play and any of their employees and associates from any and all claims from any emergency situation involving my Paw.
Guidelines and Policies
RESERVATIONS: Reservations are required with availability based on a first come, first serve basis. You may make several reservations at one time to guarantee space is available. Boarding Paws will automatically have daycare enrollment. CANCELLATIONS: We accept cancellations up to 24 hours in advance. If we are not informed of a cancellation before noon the day before, it will be considered a no show and you will be charged. The charge will be applied as a credit to your account for future use good for use one year from the date it is applied. LATE POLICY: We close at 6:30PM Monday through Friday and 4:00PM Saturday for Daycare. If you are late picking up your Paw, a fee of $1 per minute will be charged. Paws who are boarding with us must be picked up by the close of business check-out day. If you know in advance that you will be past 6:30PM, please call us so we may make the appropriate arrangements. Last pick up is 9:00PM, no later. After 9:00PM your Paw will be boarded and appropriate charges will apply. FOOD/MEDICATION GUIDELINES: All Paws are required to come with his or her own food to prevent upset stomachs by rapidly changing their food. We will feed your Paw and/or administer simple medication for you. Please label your food or medicine with your Paw’s name and directions and ensure directions are verbally communicated. SICK PAW POLICY: We reserve the right to refuse a Paw if he or she is flea infested or sick. If your Paw becomes ill while in our care, we will isolate him/her, and take care of them until we can contact you to pick him up. If your Paw is left and we find they have fleas, we will bathe and treat for fleas. Your account will be charged. COLLAR GUIDELINES: All Paws do not wear their personal collars. A collar with their name will be provided and all walks will be on a quick release collar. LEASH GUIDELINES: We follow the leash laws of Cleveland. Every Paw arriving or leaving our facility must be leashed.
